Jon Thornham
I started GST about 18 months ago. While I had several aches and pains from driving a desk for a number of years all but one have gone away. Over the last seven years, I've had post tib pain at the back of medial malleolus. I've gone to numerous PTs, had x-rays, MRIs, etc and while there is nothing wrong, the pain does not go away. I'm 5'11 at 170lbs. Today, I've been listening to Coach Sommer's podcast with Tim Ferriss and I can't help but think there is a way to fix my issue. The overarching theme is that tissue has to be properly prepared. My issue started years ago when I started running. Clearly, I did not prepare the tissue and I've paid for it for nearly a decade. I'm not expecting someone to have an answer to my problem and I know that the issue could be cause by a number of things. However, I couldn't help but wonder if someone could point me in a direction that may help. I'd be willing to do about anything to resolve the pain I have and get back to feeling like my post tib isn't about to snap at any minute.
Alessandro Mainente
I'm sorry about your situation. Btw you and all the people should understand that beside Forum Coaches who has background with human physical problems, generally we do not have the answer for every problems, we do not represent an expert team of orthopedics. Your case seems to be complex and also for a physio in the web it is hard without a face-to-face evaluation. the more it is complex the more is necessary a continue evaluation of the problem. Wesley Tan
Hi Jon, What makes you think posterior tibialis is the problem? I mean, if all the scans reported nothing abnormal who diagnosed posterior tibialis. Pain behind the medial malleolus makes me think the problem is most likely the talocrural (ankle) joint. Of course I'm speculating and haven't seen you. When does it hurt? What movements are painful and what are not. I would say start the courses. You should be fine to begin with Fundamentals and then Foundations. Front splits stretch course is also great for the ankle and achilles and the early progressions in the Single Leg Squat element of Foundations will highlight any issues in the legs generally and give you a programm to follow and begin strengthening the entire lower extremity. Go for it!

Go for it! Thanks for the reply. The pain is continual. It always aches. After hiking or running the pain becomes worse. Deep squats can cause it to flare up as well. I've been doing the stretch series and Foundation for about 18 months. I believe its a post tib issue because the pain will move up the leg to the post tib connection at times. The PTs I've worked with have all identified it as a post tib issue as well.
Wesley Tan
Hi Jon, If it was purely a muscular issue, a strain or tear of some sort than it should have healed a long time ago and I assume you have tried a period of relative rest. I still think there is likely a problem with the ankle/foot/knee as joints and muscle don't work in isolation and something must be driving the irritation. I mean, a slight subluxation can cause continual pressure on a muscle or tendon that is supporting the joint. What started it off in the first place? I mean, a slight subluxation can cause continual pressure on a muscle or tendon that is supporting the joint. What started it off in the first place? The issue first started years ago. I was running cross country and I had mild knee pain. I was put in orthotics. After years of using them the foot had basically shut off. Over a long period I rehabbed the foot and got myself out of the orthotics. At that time I started to run again and the post tib pain started. That was about 7 years ago. I've tried countless things but have never been able to get rid of the pain. Everything about GST makes sense to me. Like I mentioned a number of other minor aches and pains have gone away over the last 18 months. I believe their is a way to fix this issue, I just have found it yet.
