Diane Jacobs a PT that I have a great deal of respect for has been writing an easy to read series on patient interaction and education.
The last part in particular is very good, very simple, and can be applied not only to treatment but self treatment / prehab / training. This is a big part of what training is after all, grooming the nervous system.
