At the top of the movement, what is meant by bringing the shoulders forward?  Does this mean bend your ankles &  knees more so that your torso remains parallel to the floor but moves closer to the wall nearest your feet?  Or is it that you're supposed to press your chest towards the ceiling?

The above is exactly right. Keep your chest open/shoulder blades together as you do so.

Oh!  Thanks!  The shoulder blades together was another aspect I'd missed entirely!
