Myofascial Release Protocol?

Does anyone have or know of a good myofascial release protocol? I know how to do it, but that's all I can find is the technique. Is there a best practice as far as how often, what intensity, etc? Normally just doing whatever has worked in the past, but I've got a particularly stubborn knot that refuses to go away.

If you Google "limbering 11" you can find DeFranco's warm up that includes some foam rolling work. It's been working well for me.
